Frequently Needed Forms

New York State TAP Application
Go to www.hesc.ny.gov/content.nsf/SFC/Apply_for_TAP_Now and create an account. Follow the online instructions. The TAP code for Roberts is 0640.

Direct Loan Master Promissory Note (MPN)
If you are a first time borrower or do not have a valid MPN, this is where you'll promise you'll repay your loan(s). Go to studentloans.gov and complete "Master Promissory Note." E-sign and print a copy of your MPN and confirm your acceptance. We'll be notified electronically when you've completed this.

Direct Loan Entrance Counseling
If you are a first time borrower, go to studentloans.gov and complete "Entrance Counseling" for undergraduate. Print your confirmation page. We'll be notified electronically when you've completed this.

Federal Parent PLUS Loan Credit Check and MPN
Your parents/guardians need to complete this no earlier than 90 days prior to the beginning of the semester. If approved, you must also have a Federal Direct PLUS MPN. Go to studentloans.gov to complete the PLUS request process.

Perkins Loan
We'll email you with information prior to the start of the fall semester.

PJ Appeal for Additional Federal Assistance
Complete this form and additional required documentation if the student or parent would like to request a reconsideration of federal aid due to a change in the family's financial circumstances or there are special circumstances not considered. Additional documentation may include parent and/or student W-2 forms, written explanation of financial circumstance, and additional documentation.

TEACH Grant
Students who are receiving the TEACH grant must complete Entrance Counseling and your Grant Agreement to Serve here: www.teach-ats.ed.gov

Verification Documents for Financial Aid at Roberts

You will need to fill out these documents to verify specific information. Choose your form from the drop down box below. Below the drop down form are instructions for other documents that need to be completed that are not provided by Roberts Wesleyan College.

Student's Tax Return Transcript or Data Transfer from IRS
The student must either return to the FAFSA and use the IRS Data Retrieval Tool, or print their tax return transcript from www.irs.gov.

Spouse's Tax Transcript
The student's spouse must provide their '13 tax return transcript if they did not file jointly or did not use the IRS Data Retrieval Tool.

Parent's Tax Return Transcript or Data Transfer from IRS
A parent must either return to the FAFSA and use the IRS Data Retrieval Tool, or print their tax return transcript from www.irs.gov.

Student "NON TAX FILER" info
The student must complete the income section of the Verification Worksheet even if not filing a federal tax return.

Student's original W2s (copy)
If the student will not file, but had income in 2013, the student must provide copies of all W-2's received.

Parent's "NON TAX FILER" info
The parent must complete the income section of the Verification Worksheet even if not filing a federal tax return.

Additional Documents & Instructions for Financial Aid at Roberts

Copy of Parent's US Citizenship Document
Bring proof of citizenship to the Office of Financial Aid. Must be an original document.

Copy of Parent's Social Security Card
Information reported on the FAFSA does not match with the Social Security Administrations records.

Copy of Permanent Resident Card
Bring proof of citizenship status to the Office of Financial Aid. Must be an original document.

Copy of Student's Social Security Card
Information reported on the FAFSA does not match with the Social Security Administrations records.

Corrected Student Aid Report (SAR)
Go to www.fafsa.ed.gov as there is an error on the student's FAFSA that must be corrected.

Documentation of US Citizenship document
Bring proof of citizenship to the Office of Financial Aid. Must be an original document.

FAFSA
Complete your FAFSA at www.fafsa.ed.gov as soon as possible.

High School Name
Information regarding your High School education was left blank on the FAFSA. You must return to the FAFSA and complete the missing information at www.fafsa.ed.gov

Independency Request
The student answered questions on the FAFSA which indicate that you may be considered independent. Complete this form and attach required documentation for review

Independency Review (Completed by RWC)
The documentation submitted by the student to request Independency requires review by RWC.

Low Income Verification Letter
If the parent is likely to be credit approved, but cannot afford a PLUS loan payment, the parent may complete the PLUS loan override request by providing additional income regarding income and debts for review by the Office of Financial Aid.

Max Pell Warning
If the student is close to or has exceeded the maximum PELL grant eligibility, contact the Office of Financial Aid.

NSLDS Max Loan Warning
If the student is close to or has exceeded the maximum loan eligibility, contact the Office of Financial Aid.

NSLDS Status Resolution
If the student is currently in bankruptcy, has a loan default or overpayment, or had a loan discharged, additional information will be required. Contact the Office of Financial Aid.

Parent signature on FAFSA
For dependent students, a parent's signature is required on the FAFSA at www.fafsa.ed.gov. You can also complete it here.

Verification of Selective Service Registration
If you are male, and are 18 through 25 years of age, to receive aid you must be registered for Selective Service at www.sss.gov. Contact the Office of FInancial Aid for further information.

Unusual Enrollment History review
The Dept of Education is requesting RWC to review your enrollment patterns to determine your eligibility for federal aid. There's nothing further for you to do.
