Medical Records Retention/Authorize to Release Medical Information

New York State Law requires medical records to be kept on file for six years after a student graduated or departed from an educational institution. All medical records are destroyed after six years.

Since 1994, immunization records have been electronically maintained and may be released to former students when a signed "Authorization to Release Medical Information" form is completed and a $5.00 processing fee has been submitted to the Health Center.

As soon as payment is received, your immunizations record will be released to the designated recipient.
