Dear New Undergrad Student,
Welcome to Roberts Wesleyan College.
For college entrance and attendance, it is vital for you to complete and submit the Health History form and insurance information by August 1st, 2010.
NY State Public Health Law 2165 requires all students taking 6 credit hours or more to show protection against measles, mumps and rubella through documentation of 2 doses of measles vaccine and one dose
each of mumps and rubella. Titer Tests (blood tests) showing proof of immunity from these diseases are also acceptable. Persons born prior to 1/1/57 are exempt from this requirement. (Page 4 of the Health History form)
NY State Public Health Law 2167 (REGARDLESS OF AGE) requires either proof of meningococcal vaccine or a signed declination statement. (Page 2 & 4 of the Health History form)
Your physician, school, and previous college are often the best source for immunization records. Also, military and employment records are acceptable; however, these records may not satisfy all requirements.
It is RWC policy that all new students undergo a physical exam by their primary care doctor before beginning college (see page 3 of the Health History form).

If you have not presented the required documentation to Health Center by 30 days after the start of the semester, you will be excluded from college premises as mandated by NYS Public Health Law 2165 & 2167.
Insurance
Roberts Wesleyan College requires all full-time students to have health insurance coverage.
You are automatically enrolled in the college policy and charged each semester for health insurance. In order to waive this coverage, you must verify coverage upon entrance and every subsequent Fall semester.
All students must visit the following website to either:
1. Waive your insurance - or -
2. Purchase the college's personal insurance
Markel Insurance/Pomco
The college health insurance policy provides minimal protection. Please note that this policy is not intended to replace any coverage the student already has in place.
Canadian and other international students should carefully check the extent of their medical coverage while in the U.S. Each international student is required to purchase RWC accident and health insurance coverage along with medical evacuation and repatriation coverage UNLESS the student already has comparable coverage. Proof must be provided to the Health Center (Canadian students are not required to purchase medical evacuation and repatriation insurance.)
To ensure confidentiality, please submit the Health History form directly to the Health Center via mail, email or fax to (585) 594-6920.
If you have any questions, please feel free to contact the Health Center at (585) 594-6360.
The Health Center Administrative Office is open from Monday through Friday, 8:00am to 5:00pm to assist students with appointments, immunizations record and insurance inquiries.
