Making an Appointment

This link will be ready by Friday, September 3rd. In the meantime you can call x6882 or stop by our office (Upper VAC) to set up an appointment. We sincerely apologize for any inconvenience.

Once you are finished filling out this form someone will be in contact with you within 24-48 hours to schedule your first appointment.

If you have any questions please call 594-6882 or stop by the Wellness Center (Upper VAC) and talk with one of our staff members.

If you are filling out any of these forms out after business hours (9:00 - 5:00 pm Monday - Friday) they will not be processed until the next business day.

If you are in an urgent or crisis situation on campus call Security at 594-6170 or notify your RD.
